WAIVER & RELEASE OF LIABILITY *

By signing below, I understand and agree to the following: Voluntary Participation: My child is voluntarily participating in the Midwest Saints tryout. I understand that baseball involves physical activity and potential risks. Assumption of Risk: I accept full responsibility for any injury or illness that may occur during the tryout, including but not limited to being hit by a ball or bat, falls, or heat-related issues. Medical Authorization: In case of emergency, I authorize Midwest Saints staff to seek medical attention for my child. Release of Liability: I release Midwest Saints, Elite Athletics STL, LLC, coaches, volunteers, and facilities from all liability for injuries, losses, or damages related to participation. Health Assurance: My child is in good health and able to safely participate. I’ve shared any relevant medical concerns with staff. Team Selection: I understand that tryout participation does not guarantee team placement. Photo/Video Release: I give permission for Midwest Saints and Elite Athletics STL, LLC to take and use photographs or video of my child during tryouts for promotional or social media use.
